 ment necessary. Please check the lists below to ensure you are eligible and

 t="_blank" rel="noopener noreferrer">RSVP Now</a></p><h3 class="subhead2">A
 bout Passport-palooza</h3><p>Passport-palooza is a one day only, no appoint
 ment necessary, passport processing event open to all. It's a twice-a-year 
 event held in January and May and is for:</p><ul><li>First-time adult passp
 orts</li><li>Adult passport renewals where the original passport is expired
  by more than five years</li><li>Lost or stolen adult passport renewals</li
 ><li>New and renewing children's passports</li></ul><p><strong>If you are a
 n adult whose passport is expired by less than five years, you must renew o
 nline.</strong> Use the <a href="https://travel.state.gov/content/travel/en
 /passports/have-passport/renew.html">US Department of State's Renew by Mail
  option</a> for recent renewals.</p><h3 class="subhead2">What to Bring</h3>
 <p>You will need to bring many things with you in order for us to be able t
 o process your application. Please double check this list to ensure you've 
 got it all! We will have to send away any applicants that cannot provide al
 l of the required materials.</p><ol><li><strong>Everyone applying for a pas
 sport. </strong>Children must be present to get their passport. Both parent
 s must also be present if the child is under 16 (or you must bring a <a hre
 f="https://eforms.state.gov/Forms/ds3053.pdf" target="_blank" rel="noopener
  noreferrer">Statement of Consent (DS-3053)</a> or <a href="https://eforms.
 state.gov/Forms/ds5525.pdf" target="_blank" rel="noopener noreferrer">State
 ment of Special Family Circumstances (DS-5525)</a>)</li><li><strong>An unsi
 gned, but completely filled out, accurate <a href="https://travel.state.gov
 /content/travel/en/passports/how-apply/forms.html" target="_blank" rel="noo
 pener noreferrer">passport application (DS-11)</a> for each applicant. </st
 rong>If you are filling out the application by hand and make a mistake, you
  MUST start over. No cross outs, scribbles, or other marks can be on the ap
 plication. If you are reporting your passport as lost or stolen, you must a
 lso complete an <a href="https://travel.state.gov/content/travel/en/passpor
 ts/have-passport/lost-stolen.html" target="_blank" rel="noopener noreferrer
 ">additional form (DS-64)</a>.</li><li><strong>Original forms of ID and cit
 izenship. </strong>There are <a href="https://travel.state.gov/content/trav
 el/en/passports/how-apply/identification.html" target="_blank" rel="noopene
 r noreferrer">many documents that qualify as ID</a>, but typically these ar
 e a driver's license or your 5yr+ expired passport. There are also <a href=
 "https://travel.state.gov/content/travel/en/passports/how-apply/citizenship
 -evidence.html" target="_blank" rel="noopener noreferrer">many documents th
 at qualify as citizenship evidence</a>, but typically this is a birth certi
 ficate.<strong> </strong>Birth certificates must be original and government
  issued--if it has baby footprints on it or was issued by the hospital, tha
 t is not a birth certificate.</li><li><strong>Certified name change documen
 tation.</strong> If your name has changed at any point, whether by marriage
 , divorce, or court order, you must bring a certified copy and regular copy
  of the documentation.</li><li><strong>One black and white photocopy, front
  and back, of each form of ID and citizenship. </strong>Photocopies must be
  black and white, not color.</li><li><strong>A photo of each applicant. </s
 trong>These photos have <a href="https://travel.state.gov/content/travel/en
 /passports/how-apply/photos.html" target="_blank" rel="noopener noreferrer"
 >strict requirements set out by the State Department</a>. It must be a 2" s
 quare, color photo, with a white background, without eyeglasses or hats, an
 d a neutral expression (you can smile, but no teeth).</li><li><p><strong>Tw
 o checks or money orders per application.</strong> Each application will be
  mailed out with a check to the department of state, so you may not combine
  the cost of multiple applications into a single check. If multiple family 
 members are applying, the processing fees to Jefferson County may be combin
 ed into one check, but you will still require one additional check per appl
