Free COVID-19 Testing Registration for SEPTEMBER 23, 2020
To address the COVID-19 pandemic Jefferson County Public Health (JCPH) will be providing free COVID-19 testing. The following may be symptoms of COVID-19: Fever, Chills, Fatigue, Cough, Sore Throat, Shortness of breath or difficulty breathing , loss of taste or smell, muscle or body aches, headache, congestion or runny nose, nausea or vomiting, or diarrhea.
Please complete this sign up for each person who wishes to get tested. You do not need to have symptoms to be tested.
This round of testing will happen on September 23, 2020 from 2:00 pm to 5:30 pm at Mile Hi Church (9077 W. Alameda Ave., Lakewood, CO 80226). Please enter the testing site on Garrison street. (See testing site map below).
By completing this sign up form you are voluntarily agreeing that you would like members of the JCPH team to perform a Nasopharyngeal (NP) swab test for the Coronavirus (nasal swab test). JCPH is providing testing at no cost to the community. Signing up to receive a test does not obligate you to participate; however, if you are unable to participate at your scheduled time, please cancel your reservation.
Testing is for ages 12 and above. If you are under the age of 18, a parent of legal guardian must register and accompany you on the day of testing.
Please understand that your test sample will be sent to the Colorado State Lab for processing. Results may take 4-7 business days.
Event registration will close at 8:00 p.m. on Tuesday.
