TOURNAMENT AND ENTRY DETAILS

• Team Entry Fee is $225. Entire Team Entry Fee must be paid at time of registration..
• Minimum 4 players, maximum 8 players per team
• Divisions are based on age groups identified on registration form, however, depending on the number of teams registering, games may be played across divisions.
• Each team is guaranteed three games during round robin play.  Check in at the Evergreen Lake House is Monday, December 31st, 2014 at 7:00am. Games will begin at approximately 8:00am.
• Some divisions are already filling fast and may be closed to registration depending on overall registration in the event.  ENTRY IS FIRST-COME, FIRST-SERVED!
• To reserve a spot in the tournament please complete Drive Smart Pond Hockey Registration and Roster Form and submit with full payment of $225. Registration will not be confirmed until payment is received.

WAIVER - MUST BE PRINTED, SIGNED AND COMPLETED BEFORE ANY PLAYERS ARE ALLOWED TO PARTICIPATE. You can bring this with you the morning of the tournament.
