Mt. Evans Home Health and Hospice introduces Mountain Journey

Mountain Journey is a new program provided by Mt. Evans Home Health and Hospice that offers encouragement, support, and education to individuals and families coping with challenges stemming from serious illness. There are no fees charged to participants, because the program is community funded through grants and donations.

A Mountain Journey counselor assesses clients’ needs and identifies resources that could be helpful, and assists patients and families in communicating effectively with physicians and other care providers. Volunteers offer emotional support, companionship, and many other services. Mt. Evans’ goal is to improve Mountain Journey participants’ quality of life by addressing both the medical and non-medical issues they face as a result of their illness.

Mt. Evans Home Health and Hospice delivers professional, compassionate in-home medical care to mountain-area residents who are recuperating from an injury, living with a chronic illness, or facing end-of-life issues. A highly trained, caring staff also helps patients and family members by providing counseling services, support, and education.
