RN Palliative Care Coordinator Wanted

RN Palliative Care Coordinator wanted 30-40 hrs/wk. Coordinate patient care in the Palliative Care Program from referrals to admissions. Conduct admission visits and manage case load by taking calls, coordinating with physicians, scheduling and checking in with patients. The ideal candidate will be a licensed Certified Hospice and Palliative Nurse (CHPN) or will acquire certification within 6 months of hire (at our expense) and have 3 to 5 years’ experience in Hospice or Palliative Care.
