Athletics physicals tomorrow 8/6 at Conifer High School!!
To minimize numbers & ensure social distancing, teams will report as follows:
4:00-4:30 Football
4:30-5:00 Volleyball
5:00-5:30 Cross Country
5:30-6:00 Tennis/Golf/Soccer
6:00-6:30 Softball
6:30-7:00 Any other sports
Per doctor’s request; please have your athlete wear a T-shirt, athletic shorts, undergarments and slip-on shoes/sandals/flip-flops to cut down on time. For safety purposes, volunteers will sanitize stations & exam rooms.
Physical Examination Forms will NOT be available at the event. Please print & fill out the 3 required forms from the link below BEFORE arrival.

The fee is $20 per athlete and will be added to your student’s Jeffco Connect account. We apologize, we can’t accept any form of payment at this time.
Athletes will be responsible for retaining & uploading their forms to the athletic registration page. We can no longer keep paper forms on hand.
