Fees, taxes, it's semantics. The effect is the same for everyone, everyone will be paying more for healthcare and the industry will be less efficient than ever because it's run by the Gubment.

Probably, but, the greatest burden will fall squarly on the shoulders of middle class folks - it always does.

Of course. I don't believe there has ever been a government run program that has not been rife with waste, inefficiency, and duplication of effort.
Social security, food stamps, you name it. Once the government gets its grimy paws in something you can count on more waste, more inefficiency, and fewer freedoms. But, some people will never learn and these same people will again vote for Obama and more government control.

SCOTUS addressed fees and taxes in the recent ACA ruling using Article 1 Sec 8 clause 1 which gives Congress the powers to tax whether it is called a fee or tax.

Hang on a moment! How about these highly successful Gubment organizations:

Amtrack! Oh wait, no that's not it... The USPS! Oh wait, no they are going bankrupt... The Department of Education! Hm, well maybe not, kids don't seem to be getting very good grades in math and science these days... The DEA, Secret Service, ICE, Homeland Security, SCOTUS, the Attorney General? No, no that's not it either... Hang on, I've got it - the IRS! Everyone loves the IRS and they are very good at what they do!
